If you would like to continue your coverage after you graduate and your ACE Student Insurance Plan terminates (this year’s plan terminates August 31, 2008), please complete and submit the Continuation of Coverage Privilege Form to Janet Swain, 154 IEI Building, Notre Dame, IN 46556. Continuation is available for up to 6 months and costs $130.00 per month of continued coverage (see p. 40 in Summary Plan Description).
